CO2 laser resurfacing is the most powerful tool I have for turning back sun damage and deep wrinkles in a single treatment, and it's also the one I'm most careful about recommending, because the downtime and risk are real. Here's how I actually think through whether it's worth it for you.
What CO2 laser resurfacing actually does
CO2 resurfacing is ablative, meaning the laser vaporizes damaged surface layers of skin while heating the deeper tissue to drive collagen remodeling. That combination, removing damaged surface skin and stimulating new collagen underneath, is what produces such dramatic results in one treatment, compared to gentler options that only work at the surface or need a series to add up to the same effect.
What it fixes best
CO2 resurfacing is best for deep wrinkles, significant sun damage, acne or surgical scars, and textural changes that gentler devices haven't been able to touch. If your concerns are milder, such as early fine lines or mild uneven tone, CO2 is overkill, you'd be taking on real downtime and cost for a result a gentler laser or IPL can also deliver.
The real downtime
This is the part patients underestimate. Expect one to two weeks of raw, crusted, peeling, red skin as it heals, plus strict sun avoidance during that time and for weeks afterward. I want you to go in with realistic expectations about recovery, not just the results.
Skin-tone risk
The risk of post-inflammatory hyperpigmentation, dark patches that develop as the skin heals, rises meaningfully in medium-to-dark skin tones (Fitzpatrick IV through VI). For those patients, I often steer toward a non-ablative or fractional option instead, where the correction is more gradual but the pigment risk is much lower.
Cost, results longevity, and session tradeoffs
CO2 resurfacing costs more upfront than gentler lasers, but because it stimulates deep collagen remodeling, results can last years rather than months. Gentler options are cheaper per session but usually need a series to reach a comparable result, so the real cost comparison has to include how many sessions you'd actually need.
When it's worth it
